Dr. John A. Kastor has studied two leading centers in specialty care, the Cleveland Clinic and the University Hospitals of Cleveland, to learn what these institutions are doing to survive in the current era. Using the findings of more than two hundred interviews with physicians, administrators, investigators, and trustees, the author describes in detail these rival organizations, their individual struggles against the economic pressures presented by managed care, and their sometimes bitter competition for patients.

It has only been a short time since the existence of suppressor cells was demonstrated, yet in this period of time a great deal of information has been compiled concerning their basic science and their functions or disordered functions in human disease. This volume is an tempt to clarify the immune mechanisms through which these cells act, their normal functions, and how their functions are disordered in certain disease states. In this volume some rational approaches are proposed to therapy of diseases perceived at least partially due to disordered immune regulation. Particular focus is on the recently emerging field of soluble suppressor factors as a means of providing adoptive immunotherapy in certain situations. I hope that future work in this field will justify the confidence in this approach.

Ever since civilised society began, we have felt the need to classify, categorise and specialise. It can make things more efficient, and help give the leaders of any organisation a sense of confidence that they have the right people focusing on the right tasks. But it can also be catastrophic, leading to tunnel vision and tribalism. Most importantly it can create a structural fog, with the full picture of where an organisation is heading hidden from view. It is incredibly widespread: the chances are these 'silos' are rife in any organisation or profession, whether your business, or your local school or hospital. Across industries and cultures, as this brilliant and penetrating book shows, si...
